Canisius College

Students who are ready to receive their degree must:

• Download the Application for Graduation/Request a Diploma Form (Adobe Acrobat 13.0 KB)
• Return the completed form to the appropriate office listed below.

If you need a replacement diploma because your original was lost or damaged, download and submit the Replacement Diploma Request Form (Adobe Acrobat 12.0 KB) with the $30.00 replacement fee.  Be sure to include the degree and date the degree was awarded.  Make checks payable to Canisius College. It takes approximately two weeks to receive the diploma.  Send the request to the Student Records Office at the address below.
